Intensive Wellness & Support Program
The Intensive Wellness & Support Program at Hope Seed provides concentrated, individualized support for children, adolescents, adults, and families who need more structure, consistency, and hands-on guidance than traditional weekly services alone may provide.
Rather than focusing on a single diagnosis or challenge, the program is built around the whole person and the realities of everyday life. Support is individualized to each client’s needs and may include emotional regulation, coping and resilience skills, supported meals and food-related routines, behavioral support, family involvement, confidence building, daily living skills, school or community reintegration, and strategies for navigating chronic illness, medical complexity, or significant life transitions.
The goal is not simply to help someone function within a structured environment. It is to help them carry what they are learning into real life.
Through Hope Seed’s Recovery Continuum, concentrated support is gradually transitioned as stability, confidence, skills, and independence increase. Clients and families are supported in developing sustainable routines, strengthening communication, recognizing challenges earlier, practicing effective responses, and building the self-leadership needed to move forward with greater confidence.
For individuals already receiving medical, psychiatric, nutritional, or other clinical treatment, the program can also provide an additional layer of practical and whole-person support while collaborating with the client’s established care team when appropriate and authorized.
The destination is not dependence on support. It is greater confidence in living beyond it.
Phase I | Weeks 1–6
Individualized Recovery & Wellness Intensive
The first six weeks create concentrated structure around the client’s emotional, behavioral, relational, nutritional, and
practical recovery needs.
2 private 60-minute sessions each week
Individualized coaching, psychoeducation, regulation work, reflection,
and recovery-focused skill building.1 weekly skills /experiential component
Real-life application selected around the client: social situations,
planning, identity work, creative exercises, values, routines, or other
appropriate practice.
1 caregiver component each week
Education, communication coaching, routine support, and strategies
that help the home environment reinforce recovery.
2 structured check-ins each week
Brief scheduled touchpoints for accountability and skill reinforcement.
Individualized recovery work.
Curated presentations, worksheets, videos, reflection prompts, family
activities, and between-session practice.
Beginning, midpoint & transition review.
A defined care map that is reviewed and adapted as the client
progress.
The Hope Seed Integrated Support Areas
Each individualized care plan can incorporate five connected areas of support.

Routines, actionable goals, and guidance that create direction and consistency.
5. Identity, Purpose & Future Planning
Clarify values, strengthen identity, and build meaningful future chapters.

Phase II | Weeks 7–12
Transition & Independence Track
The second six weeks intentionally change the relationship with support. Sessions move to once weekly while the client
practices using skills with less prompting and greater confidence in real-world settings.
1 private 60-minute session each week
Focused application, problem-solving, setback awareness, and growing self-leadership.
Tapered between-session contact
Weeks 7–8 may retain one structured check-in; prompting reduces further as independence grows.
Caregiver transition support
Guidance shifts from active structure toward observation, communication, reinforcement, and knowing when additional
help is needed.
Continued individualized preparation.
Sessions remain curated around progress, current challenges, outside-provider recommendations, and transition goals.
Graduation/maintenance map.
The final weeks produce a personalized long-term support map, warning-sign plan, and recommendations for the next
appropriate layer of care.

Hope Seed offers multiple levels of individualized support. Payment plans are available. Recommendations are based on the individual’s needs, goals, readiness, and appropriate scope of care.

INDIVIDUALIZED & SUPPLEMENTAL SUPPORT SERVICES
Individual services may be selected based on the client’s current needs and may also be used to supplement existing outpatient, IOP, PHP, medical, nutritional, or therapeutic care.
CORE INDIVIDUAL COACHING SESSION — $170
45–50 minutes of individualized coaching in behavioral, emotional wellness, recovery support, or personal development.
DEEP TRANSFORMATION SESSION — $250
90-minute extended session designed for deeper work on patterns, identity, coping, emotional regulation, trauma-informed support, and significant life transitions.
FAMILY DYNAMIC SESSION — $225
60-minute parent, caregiver, or family session focused on communication, support strategies, boundaries, family dynamics, and creating a more supportive home environment.
SUPPORTED MEAL PREPARATION & FOOD CONFIDENCE SESSION — $200
Individualized support focused on meal preparation, food-related anxiety, routines, confidence, and practical skills within the scope of coaching and behavioral support.
EXPERIENTIAL SUPPORT SESSION — $175 + OUTSIDE COSTS
Structured, real-world coaching experience designed around the client’s individual goals, needs, and established care plan.
Outside-provider, activity, admission, transportation, or other experiential costs are billed separately when applicable.
STABILITY & SUPPORT CHECK-IN — $85
20–25 minute focused session providing accountability, coping support, reinforcement of established strategies, and support around current needs.
CARE COORDINATION — $50 / 30 MINUTES
Communication and collaboration with authorized members of the client’s established care team when appropriate. This may include therapists, physicians, dietitians, IOP/PHP programs, schools, or other approved providers.
IMPORTANT INFORMATION
Hope Seed services are individualized and are not a substitute for medical care, psychotherapy, nutritional treatment, or a clinically indicated higher level of care.
When appropriate, services are designed to complement and coordinate with the client’s existing treatment team.
Recommendations are based on the individual’s needs, goals, readiness, and appropriate scope of care.
Experiential services: Outside-provider costs vary by modality, provider, availability, and client need. When an outside provider bills separately, those fees are the client’s responsibility unless otherwise stated in the agreement.

Hope Seed’s Recovery Continuum begins with concentrated individualized support and intentionally transitions toward
greater independence. The purpose is to help clients use skills, routines, family support, and self-leadership more
confidently in everyday life.

decreasing reliance on check-ins and increasing

Why taper? The second phase is a planned reduction in support. Clients are encouraged to recognize needs earlier, choose tools more independently, use their existing treatment and family supports appropriately, and prepare for life.
Supported Meal Preparation & Food Confidence
Hands-on support designed to help the client practice planning, preparing, and participating in meals in a supportive environment. Sessions focus on building independence and confidence around food-related routines while practicing coping strategies for emotions, avoidance, control, food aversions, and discomfort that may arise before, during, or after eating. Skills practiced through Hope Seed are intended to complement—not replace—the client’s IOP treatment plan or nutrition recommendations.

Weeks 7–12 | Practice Independence
Week 7- Stabilizing the transition
Review what is holding, what feels vulnerable, and what support is still needed as intensity
decreases.
Week 8 - Independence in real life
Apply skills to food, medical, school, family, social, travel, restaurant, or routine challenges.
Week 9- Less prompting, more self-leadership
Identify the problem, name available tools, and choose a response before the coach steps
in.
Week 10- Setback planning
Distinguish a hard day from an emerging pattern and practice responding early rather than
waiting for crisis.
Week 11- Building the long-term support system
Clarify the roles of family, medical and nutrition providers, behavioral-health professionals,
school, peers, and self-support.
Week 12- Graduation & continued-care map
Review progress, warning signs, coping strategies, support contacts, and the appropriate
plan after the pro
